You can sign up and register for the Patient Portal by giving your provider your name and email address. In turn, your provider will give you an instruction sheet that includes an activation code, known as an enrollment token. The enrollment token will allow you to log in to the system and create your own private user name and password.
Or, by providing your name and email address you can be automatically enrolled with your temporary user-id, password and security question emailed to you. Then just log on and select your permanent user name, password and security question.
