Patients’ Rights and Responsibilities

The patient is responsible for:

  • Giving accurate and complete information about complaints, past illnesses, hospitalizations, medications, advance directives, and other matters of care.
  • Telling the staff or doctor when you do not understand a treatment course, use of medication, or care decision.
  • Giving correct up-to-date insurance information name, Social Security Number, address, and treating physicians.

The patient has a right to:

  • Review the record of your case at any time.
  • Refuse all or part of your treatment and be informed of the expected results of that decisions.
  • Be informed of the relationship between this practice and any other organization or service to which you are referred. You have the right to choose the organization or service you prefer.
  • Receive service without regard to race, color, sex, national origin, religion, age, or handicap.
