If you are ever unable to make your own health care choices or to clearly communicate your wishes, other people will have to make choices for you. Advance Directives are a way to let your family, friends, and health care providers know, in advance, of your wishes to receive, or not receive, medical care and treatment.
To make sure that your family, friends, and health care providers understand your choices, it is important to have your Advance Directives in writing. GCS staff will ask you if you have advance directives; we also have forms and more information available if you need.
